Currently Club Smile collects and retains personal identifying and payment information for the business of servicing customer teeth whitening subscriptions. ClubSmile reserves the right to amend this policy at its discretion on an as-needed basis, as is reasonable; in such event all customers will be notified.
These terms of service are a legal agreement between customer and ClubSmile. By using website and/or products from ClubSmile, they affirm they are 18 years of age or older, or have been given permission by their parent/guardian to do so.
There are no cancellation fees. To cancel, customer must notify clubsmileteam@gmail.com; service will be discontinued the first day of the following month, no pro-rated refunds will be offered. Returns and refunds must be made within 30 days of receipt of product in question. In the event of refunds/dissatisfaction, customer must contact clubsmileteam@gmail.com with explanation and mention of product(s) in question. ClubSmile will refund the membership fee and most recent month’s payment, less shipping to return product(s) identified as unsatisfactory. ClubSmile leaves room for other accommodations and/or full refunds to be allowable in some cases, at ClubSmile’s discretion. Unopened/unused products may be returned without notice for full refund, less shipping, as long as done within 30 days.
ATeeth whitening products used by ClubSmile are meant for natural teeth only, and can cause gum irritation, tooth sensitivity, and other possible side effects. Side effects can occur from both proper and improper use of products, intentionally or unintentionally. By agreeing to these terms, customer understands and agrees to take on those risks personally and waives any liability of those risks against ClubSmile. Always follow all included instructions and avoid product contact with anything other than teeth. ClubSmile emphasizes discontinuing use of any product causing any undesirable or unexpected effects.
ClubSmile subscriptions will be processed automatically, every month. At the end of a year, customer will automatically be sent a brand new custom tray whitening kit and billed the full membership fee corresponding with their subscription, unless they notify ClubSmile otherwise by opting-out. To opt-out: One month before the yearly renewal, ClubSmile will contact customer via email with option to opt-out of receiving a new kit, in order to pay the reduced membership fee as specified in the email. We reserve the right to discontinue service and/or leverage collection agencies in case of untimely payment of bills.
It‘s simple. You sign up by selecting a whitening plan of your choice and creating an account with your shipping address and billing details. We ship your teeth whitening box in your first order. Then we bill and ship your ongoing boxes for your every 2,3, or 4 months after, like a subscription. You can cancel anytime.
